This agreement is between, Inc. and _______________________________, the adopter, concerning the adoption of the animal described above. 

We agree to the following terms & conditions of this animal adoption:

1.     Adopter agrees that PetPals, Inc. has the right to investigate the home and care of the adopted animal at any time.  If, in the opinion of the investigators, the pet is not receiving adequate and proper care, feeding, shelter, and/or medical attention, or if there is, or has been, any evidence of cruelty or failure to observe all prevailing animal ordinances or anti-cruelty laws, PetPals, Inc. has the right to reclaim the animal immediately.

2.     Adopter agrees to notify PetPals, Inc. within 48 hours if: the pet becomes lost or stolen; if the pet dies from any cause; or the adopter changes his/her address or phone number shown below. 

3.     Adopter agrees not to sell or give the above described pet to any other person or agency unless consented to in writing by PetPals, Inc. of Putnam County.  Adopter agrees that the pet will NEVER be used for any experimental purposes nor breeding purposes.  Adopter also agrees that this animal will NEVER be used for any fighting-related purposes.  If for any reason the adopter cannot care for or keep the adopted animal, he/she agrees to return the animal to PetPals, Inc. or other agency approved by PetPals, Inc. in writing. 

4.     The known medical condition of the animal has been discussed at the time of adoption and it is understood that the new owner will assume responsibility for any and all medical costs both present and future.  While the rescuer makes every effort to place only healthy animals, the rescuer cannot guarantee the health of any animal and shall not be held responsible for any medical expenses which may be incurred, hereby expressly excluding any implied or express warranties of merchantability or fitness for any particular purpose, including, without limitation, any warranties regarding health, temperament or whether the animal is housebroken.  Adopter also agrees to not cosmetically alter the animal unnecessarily in any way, including but not limited to the declawing of cats, the docking of tails, and the cropping of ears.

5.     Adopter agrees to have the animal spayed or neutered, if not already done so, by the date of _________, 20__.  Adopter agrees to provide proof of surgery by a qualified veterinarian to PetPals, Inc. by this date, otherwise, the animal will be immediately returned to PetPals, Inc. 

6.     Adopter agrees that any sum of money given to PetPals, Inc. as an adoption fee is a donation that helps in caring for this and other homeless animals, and that this fee is non-refundable.
